J.D. tries to get through to a young patient about the dangers of smoking. Meanwhile, Elliot gets off on the wrong foot with the nurses, and Turk asks Carla out.

Quote from J.D.

J.D.: [v.o.] A hospital is a tricky place. It's very cliquey and you have to watch what you say. It's a lot like high school.
[fantasy scene: a pimply, glasses and headgear-wearing Elliot walks around the cafeteria in a high school band costume. After she trips and drops her tray, everyone laughs.]
[reality: Elliot picks up her food from the floor and sits alone at a table]
J.D.: [v.o.] I like to think, if I didn't have somewhere else to be, I would have gone and sat with her. But tonight, there's somebody else I have to take care of.

J.D.: I know that watching Will slowly kill himself hurts you, too. Now, it's even harder if you don't have anyone one to talk about it with. So, when I got off work, I figured, you know, I was going to pick up a few cold ones, right? And, you know, maybe you and I, we'll talk it out, we'll just break the whole thing down. Right? What do you say to that?
[Dr. Cox slams the door on J.D., knocking him to the ground]

Elliot: I'm sick of this. Everyone keeps ignoring me. I'm like a damn plague around here.
Turk: Please, don't look back at me when you're talking.
